The Homeless Program at Community Health Care is designed to assist individuals and families living in shelters, transitional housing, treatment facilities and living with friends or families on an emergency basis with comprehensive primary health care and case management services.
Our Outreach team, which is comprised of a medical provider, medical assistant, case manager and administrative staff, provide services to 6 outreach sites in the Quad Cities. The team travels to the outreach sites 5 days a week, specifically targeting those that are experiencing homelessness.
Referrals are made for medical specialists, dentistry, and mental health services. Case management services connect clients with the community and assists clients in developing/negotiating an action plan that promotes self-sufficiency to end homelessness.
It is the mission of our Outreach team to promote health, hope and dignity for those experiencing homelessness in the Quad Cities through accessible and comprehensive care at Community Health Care. |
