| Moline Clinic |
Community Health Care’s Moline Clinic opened in August 1994. The clinic is located in the Floraciente neighborhood in the heart of Moline, Illinois. Spanish appointment scheduling and translation services are offered on-site. From its beginning with just one physician, the practice has grown to include one internist, two pediatricians, and two physician assistants. Today, the Moline Clinic serves over 5,000 children and adults each year. The clinic also houses the Rock Island County Health Department’s Women, Infants and Children (WIC) program, a nutrition program for pregnant women, new mothers, and young children. |

| Community Health Care’s Rock Island Clinic, CHC’s newest location, began providing medical care in March of 2003. Located near schools and housing sites in western Rock Island, Illinois, the clinic serves over 3,000 adults and children each year. Services offered include well-child checkups, school physicals, and immunizations. CHC continues to grow its services at the Rock Island Clinic. The latest addition is new dental services, which were added in November of 2005. |
