| The River Drive Clinic is Community Health Care's largest medical clinic. Also the site of CHC's administrative offices, the clinic is located in downtown Davenport, Iowa, overlooking the beautiful Mississippi River. Serving 10,000 patients each year, the clinic offers preventive, acute and chronic care, including family practice, internal medicine, pediatrics, and obstetrics/gynecology services. On-site ancillary services include radiology, laboratory, pharmacy, and health education, as well as Spanish and Vietnamese translation. The Homeless Outreach Team, based at the River Drive Clinic, also serves patients at drop-in clinics located throughout the community. |

| The Community Health Care Dental Clinic, in downtown Davenport, Iowa, is conveniently located just across the street from CHC’s River Drive medical clinic. The Dental Clinic serves over 6,000 pediatric and adult patients each year. Preventive, acute, and restorative services are provided. Just as at CHC medical clinics, sliding fees are available for those who qualify. |

| Community Health Care’s Regional Virology Clinic opened in 1995. The Virology Clinic serves the HIV-infected populations in Scott County, Iowa and Rock Island County, Illinois and surrounding rural counties. Located on the west campus of Genesis Health System in Davenport, Iowa, the clinic offers testing, treatment, and counseling services as well as patient support groups. Staff members also provide outreach and education services throughout the community, to medical and educational professionals and students, and at drug treatment and youth rehabilitation facilities. |

Community Health Care’s Davenport West clinic is located at 4205 North Fairmount, Davenport, IA 52806, behind Kmart on W. Kimberly Road. The clinic is conveniently situated in the same building as the rental office and Head Start at the Fairmount Pines apartment complex. Services provided at the clinic include acute (same day) care, annual physicals for adults and children, well child check-ups (no immunizations), women’s health services and referrals to specialists. Hours for the Davenport West clinic are Mondays from 8:00 am to 5:00 pm, Tuesdays from 9:30 am – 6:30 pm and Thursdays from 8:00 am to 5:00 pm. For appointments call 563-336-3000.
