  Syed Haque, MD, Medical Director

Dr. Syed Haque joined CHC in 1996 as a staff physician, and became Medical Director in August 1999. He practices in Internal Medicine at CHC’s River Drive Clinic in Davenport.

Dr. Haque received his M.D. at Aga Khan University Medical College in Karachi, Pakistan. His post graduate training includes serving as Resident House Officer in Surgery ER & Orthopedics at Aga Khan University, Pakistan, and as Resident House Officer in Medicine, Primary Health and Community Health. He completed a three-year residency in Internal Medicine at Stony Brook University Hospital, the University of New York, Stony Brook, New York.

He served as Chief Medical Resident at SUNY, in Northport, New York and as Clinical Assistant Instructor in the School of Medicine, Stony Brook, New York.

   
   

Mirza Omer Baig , MD, Internal Medicine

Mirza Omer Baig, MD joined CHC in September of 2006 at the River Drive Medical Clinic in Davenport.  Dr. Baig earned his Bachelor of Medicine/Bachelor of Surgery degree from the Dow Medical College in 1997. He received his board certification from the American Board of Internal Medicine in 2005.

   
   

Mulumebet Haileselassie, MD, Obstetrics and Gynecology

Dr. Mulumebet Haileselassie joined CHC in 1990. She practices in Obstetrics and Gynecology at the River Drive Clinic. Dr. Haileselassie received her MD at the Haile Sellassie University in Addis Ababa, Ethiopia. She completed her Obstetrics and Gynecology Residency at Cook County Hospital in Chicago, Illinois. Prior to joining CHC, she practiced at the Chicago Department of Health and in private practice.

   
 

Linda Ozaki, MD, Pediatrics

Dr. Linda Ozaki joined CHC in 1982. She practices in Pediatrics at the River Drive Clinic. Dr. Ozaki completed Medical School at the University of Kansas and earned a Bachelor of Science degree from Washburn University of Topeka.

   
   
John T. Johnson, DO, Family Practice

Dr. John T. Johnson joined CHC in 2004. Dr. Johnson practices in Internal Medicine at the River Drive Clinic. Dr. Johnson holds a DO degree from the Philadelphia College of Osteopathic Medicine, and a Bachelor of Science in Premed, Biology, and Chemistry, from Parsons College in Fairfield, Iowa. Prior to joining CHC, he was in private practice for over 27 years.

 
   
Ram Niwas, MD, Pediatrics

Dr. Ram Niwas joined CHC in 2006. Dr. Niwas comes to CHC’s River Drive Clinic from Stroger Hospital of Cook County in Chicago, where he completed a fellowship in Neonatology Medicine. He completed his residency in Pediatrics at New York Methodist Hospital in Brooklyn, New York, as well as a fellowship in Neonatal Medicine at the State University of New York at Stony Brook. He earned his MD at S.N. Medical College in Agra, India.

 
   

Oludamilola Ogunlesi, MD

Oludamilola Ogunlesi, MD, joined CHC in June of 2007 at the River Drive Medical Clinic in Davenport. Dr. Ogunlesi earned his Bachelor of Medicine/Bachelor of Surgery from the University of Lagos in Nigeria in 1999. He continued his medical studies at the University of Maryland, College Park, receiving his Masters of Public Health In Community Education.In March of 2000-April of 2001, Dr. Ogunlesi volunteered as a physician in an outpatient clinic in Lagos, Nigeria. He conducted his residency at the Huron Hospital in East Cleveland, OH, specializing in Internal Medicine.

   
Angela Dolphin, PA-C

Angela Dolphin, Physician Assistant, joined the River Drive Clinic’s Family Practice staff in 2002. Ms. Dolphin completed her Bachelor of Science degree at Loras College in Dubuque and completed the Physician Assistant program at the University of Iowa in Iowa City, Iowa.

   
Jane Sieleman, PNP

Jane Sieleman, Pediatric Nurse Practitioner, joined CHC in 2005. Ms. Sieleman holds a Pediatric Nurse Practitioner Certificate from the University of Iowa and a Diploma of Nursing from St. Luke’s School of Nursing in Cedar Rapids. She earned her Bachelors and Masters degrees in Nursing at the University of Iowa. Ms. Sieleman brings many years of clinical experience to CHC, including over twenty years as an Advanced Practice Nurse with the University of Iowa Hospitals and Clinics, where she worked with the Iowa Child Health Specialty Clinics as the Regional Coordinator for the Waterloo Regional Center.

 
   

Joe Caccamo, PA-C, Homeless Outreach

Joe Caccamo, Physician Assistant, began work at CHC in 2006, in the Homeless Outreach program. Mr. Caccamo comes to CHC from Des Moines University in Des Moines, Iowa, where he completed his Physician Assistant studies, earning a Master of Science degree. He holds an undergraduate degree in Biology from the University of Saint Francis in Fort Wayne, Indiana, as well as an Associate of Emergency Medical Services degree from Lutheran College of Health Professions, also in Fort Wayne. He has previously worked as an Emergency Department Paramedic at Winona Memorial Hospital and at Parkview Huntington Hospital.

   
Bernie Tadda, FNP

Bernie Tadda, Family Nurse Practitioner, joined CHC in 1988. Mr. Tadda practices at the River Drive Clinic. He earned a Bachelor of Science at Creighton University in Omaha, Nebraska. He earned his Master of Science at the University of Illinois at Chicago College of Nursing.

   
Dawn Nowell, FNP

Dawn Nowell, Family Nurse Practitioner, joined CHC in 2004. Ms. Nowell practices at the River Drive Clinic. She earned a Nursing Diploma at the Moline Public Hospital School of Nursing in Moline, Illinois. She earned her Bachelor of Arts degree in Special Studies at St. Ambrose University in Davenport, Iowa. She holds a Master of Science in Public Health Nursing from the University of Illinois. Prior to joining CHC, Ms. Nowell worked as a nurse in a hospital and in a private practice.

 

Lisa Nelson, PA-C

Lisa Nelson, Physician Assistant, previously worked at CHC from 2000-2005.  In 2005 she had the opportunity to practice in England for two years at the Saxonbrook Medical Centre in Crawley, West Sussex.  Ms. Nelson rejoined the CHC River Drive Clinic in February 2008.  Lisa has been appointed the Clinical Director at CHC.  She will spend half her time as the director and the rest of her time will be spent seeing patients.  She earned her Master of Science degree at Emory University, School of Medicine in Atlanta, Georgia.  She holds a Bachelor of Science degree in Biology from the University of Iowa in Iowa City, Iowa.

 

Sonal Patel, PA-C

Sonal Patel, Physician Assistant, joined the CHC River Drive Clinic staff in November 2007.  Ms. Patel earned her Master of Science degree at Midwestern University in Downers Grove, Illinois.  She holds a Bachelor of Science and Engineering degree in Biomedical Engineering from the University of Iowa City, Iowa.

 

Steven Burke, PA-C

Steven Burke, Physician Assistant, joined the CHC River Drive Clinic in October 2007.  Mr. Burke earned his Bachelor of Science degree in Physician's Assistant from the Rutgers University in Piscataway, New Jersey.  Steven has over 25 years of experience as a Physician Assistant before joining CHC.
