Homeless Program |

The Homeless Outreach Program at Community Health Care, Inc. is designed to assist individuals and families living in shelters, transitional housing, treatment facilities or with friends/family on an emergency basis with comprehensive primary health care and case management services.
Our Outreach Team, which is comprised of a medical provider, medical assistant, case manager and administrative staff provide services to three outreach sites across the Quad Cities. The team travels to each site multiple times each week, specifically targeting those experiencing homelessness.
Referrals are made to medical specialists, dentists and mental health services. Case management services connect patients with community organizations serving the homeless and assists clients in developing/negotiating an action plan that promotes self-sufficiency to end homelessness.
It is the mission of our outreach team to promote health, hope and dignity for those experiencing homelessness in the Quad Cities through accessible and comprehensive care. |

| Nutrition Services |

Lynn Bachelder is a registered, licensed dietitian in Iowa and Illinois. She obtained her Bachelor’s degree in nutrition from Marycrest College in 1989. She also completed a postgraduate internship at the University of Iowa.
As a dietitian, she provides nutrition assessment and advice for people of all ages.
Her main focus is on CHC, Inc. adult population with Diabetes and the Diabetes Collaborative. Through the Collaborative and our regular “Diabetes Days” (Specialized Clinic days) She is able to help patients manage their diabetes through proper diet and exercise.
During a “Diabetes Day” (Specialized Clinic) our patients are educated regarding health and nutrition options that will help them better manage their disease. If a patient needs more help, they can discuss their diet issues on a one-to-one basis through a scheduled appointment. These issues include; weight loss, high cholesterol, high blood pressure or general nutrition.
One day a week she works in the Child Health clinic. She provides nutrition advice to the parents of children from birth through adolescence. Through our partnership with the Scott County Health Department, Lynn is able to certify infants and children, through their fifth year, for the WIC (The Special Supplemental Program for Women, Infants and Children) program.
Ask the Dietitian is a new program that reaches people who may not want to make an appointment for various reasons. Lynn positions herself on the second floor lobby of our River Drive Clinic and is available to answer and give nutrition information to any patient or visitor who is interested.
Keeping employees healthy is an important issue to CHC, Inc. A series of educational “Brown Bag Lunch” seminars are provided on a periodic basis. Examples of topics include healthy weight loss, fad diets, cholesterol and proper nutrition.
Lynn’s services are available at no cost to our patients. |

- Rapid Testing – Strep A, Mononucleosis, Serum HCG, Overnight plating for
Strep A
- Send-Out testing is performed at Quest Diagnostics
